Parking Ticket Appeals

The Office of the Corporation Counsel will review those appeals of parking ticket issued in error
or involving faulty meter devices. Minor technical errors, such as incorrect color of the car, will
not be grounds for dismissing a ticket otherwise issued correctly. Please fill out the form in a
complete and legible fashion. The original ticket must be enclosed with the appeal form. You
will be notified of a decision of the Corporation Counsel’s office if your appeal is denied, at
which time you will be notified by letter.

I certify that the above statement is a true and accurate description of the events cited herein. I
understand that the making of a false statement is punishable as a Class A misdemeanor
pursuant to Section 210.45 of the Penal Law, subject to a punishment of up to one (1) year in
jail and /or a $1,000.00 fine.

ALL SUPPORTING DOCUMENTATION TO SUBSTANTIATE THIS APPEAL MUST BE INCLUDED
HEREIN. FAILURE TO PROVIDE ALL INFORMATION NECESSARY TO SUPPORT YOUR CLAIM SHALL
RESULT IN A DENIAL THEREOF.
